    • A heart stimulator is disclosed which is capable of sensing and stimulating both the atrium and ventricle in a heart such a way that a ventricular stimulation pulse is emitted either after a stimulated or spontaneous atrial event following the atrioventricular interval, or after the expiration of a minimum synchronous interval following a stimulated or spontaneous ventricular event, depending on which interval elapses last. The functioning of the heart stimulator, when the atrium's spontaneous rate is faster than a stimulation rate corresponding to the minimum synchronous interval, is improved by delivering an extra atrial stimulation pulse at a second predetermined atrioventricular interval before the next ventricular stimulation pulse, if the interval between the most recently detected atrial event and the next ventricular stimulation pulse exceeds a predetermined threshold value.

    • In a dual chamber pacemaker capable of stimulating the atrium and ventricle in a heart, a stimulation pulse in the atrium could be sensed as a ventricular event which cannot be distinguished from a premature ventricular contraction, so the pacemaker institutes a blanking interval in which no sensing occurs and a crosstalk interval in which sensing occurs, detections in the crosstalk interval causing the emission of a ventricular stimulation pulse with a shortened A-V interval. A dual chamber pacemaker and a method for operating same are disclosed wherein the blanking interval is optimized so as to reduce the risk of a PVC being missed and so as also to minimize the number of detected events, caused by crosstalk from the atrium, in crosstalk intervals. This is accomplished by counting, for a number of cardiac cycles, the number of detections in crosstalk intervals and forming a relationship of the count to the number of cardiac cycles. This reduction is then used to determine the length of the blanking interval.

    • Embodiments of the present invention include a system and method for implementing a presence system. According to an embodiment of the present invention, responsive to receiving a request for presence information associated with a presentity from a watcher, the presence system receives instructions indicating that an authorization instance other than the presentity shall be given an opportunity to change or verify an authorization rule associated with the request for presence information. As a consequence, the presence system notifies the authorization instance of the request for presence information, thereby enabling the authorization instance to change or verify the authorization rule. The presence system also makes a final decision on the authorization rule on the basis of the instructions and a notification indicating a change or verification of the authorization rule.

    • Heart stimulating device comprising a stimulation pulse generator arranged in a stimulator housing, where the heart stimulating device is coupled to a plurality of stimulation and sensing electrodes arranged at one or many leads, the electrodes being adapted to be arranged in connection with the heart to stimulate the heart. The device further comprises a sensing unit adapted to sense electrical heart tissue responses to applied electrical stimulation pulses via said electrodes. The pulse generator is adapted to generate stimulation pulses to be applied to the heart tissue by different electrodes, and for each of the applied pulses resulting in a depolarization wave in the heart tissue a depolarization wave velocity parameter of the wave is determined by the sensing unit and stored in a memory unit. The stored wave velocity parameters, related to different stimulating electrodes, are compared to each other in a comparison unit and the comparison is used to adapt a stimulation mode when stimulating the heart to optimize the hemodynamic performance of the heart.

An AV conduction capacity is detected, wherein a sensed ventricular event following an intrinsic or paced atrial event during a predetermined period of time indicates good AV conduction capacity and wherein absence of a ventricular event within the predetermined period of time indicates poor AV conduction capacity. At least one A-V node function parameter indicating a function of the A-V node is determined, wherein the A-V node function parameter includes whether a status of the AV conduction capacity is good or poor. Incipient A-V node malfunction is detected where poor AV conduction capacity indicates incipient A-V node malfunction.
