    • A positionable internal retraction device is provided comprising a malleable ring member and a web-like structure. The retraction device operates to temporarily reposition tissues and organs from an operative site to provide a clear access and visual path for the surgeon. The ring member may be elongated, twisted, folded, bent or deformed to provide an appropriate insertion profile and subsequent functional shape. The retraction device may be shaped for both open and minimally invasive surgeries. The retraction device is atraumatic and may be used for retraction of delicate tissues and organs. The ring member may have different bending biases. The web-like structure may be constructed of any elastic material that can stretch and recover from the shaping and reshaping of the ring member.

    • A surgical staple-clip including a clip component and a securing member is used in a wide range of surgical procedures. The staple-clip may be introduced to a surgical site in an un-assembled condition through a small port or trocar. An applier for the staple-clip comprising a pair of opposed jaw-like channels is provided to position and apply the clip component and the securing member. The clip component is positioned around a target tissue and is compressed or clamped upon the tissue using only the force required for a specific surgical procedure such as occlusion, ligation or fixation. When the clip component is properly applied, the securing member is urged forward and over the clip component to secure the staple-clip. The clip component may include traction enhancement features such as surface interruptions, bumps, valleys and ridges. With the staple-clip of the invention, the force required to constrict or occlude the tissue is separate from the force required to secure and maintain the staple-clip in position and, as a result, the body tissue is not over-compressed and nourishment to the body tissue is maintained. Other aspects of the invention include thumb actuated clip appliers for use in hand assisted laparoscopy (HAL) In one embodiment, a clip applier includes a handle and a thumb actuated mechanism that is used to slidably release clips onto a body tissue or vessel by sliding the thumb actuated mechanism forward and backward using only one hand. In another aspect of the invention, a two-stage clip is disclosed having a clip component and a staple component for securing the clip after it has been properly positioned.

    • The invention is directed to a trocar seal adapted to form a seal around a surgical instrument, the trocar seal comprising a housing having an axis extending between a proximal end and a distal end and forming a working channel sized and configured to receive the surgical instrument; an elastomeric septum seal having a proximal face and a distal face, the septum seal being disposed in the housing and extending transverse to the axis of the housing across the working channel; portions of the septum seal defining a hole having a diameter less than or equal to the diameter of the surgical instrument so that during insertion of the instrument along the working channel the septum seal forms a seal with the instrument; and at least one structure formed integrally with or proximate to the septum seal to prevent inversion of the septum seal upon withdrawal of the surgical instrument. The septum seal or the housing may further comprise a sidewall to include the structure extending from the sidewall to the distal face of the seal to tether the seal distally. The structure may be integrally formed with the proximal face of the septum seal to reinforce the seal, or the structure may be integrally formed with the distal face of the septum seal to tether the seal distally. The structure may be an annular reinforcement or rib, and is formed so as to allow side-to-side movement or floatation of the seal. The trocar seal may further comprise a plurality of radially extending ribs formed on the proximal or distal face of the septum seal to reinforce the seal. The trocar seal may further comprise a tensile element formed on the distal face of the septum seal.

    • An endoscopic clip applier for the temporary as well as permanent occlusion of vessels and the clipping of other tissues. The clip applier includes an elongated outer body having a proximal end attached to an actuating mechanism and a distal end. An elongated inner member is slidably positioned within the outer body and has a proximal end coupled to the actuating mechanism and a distal end coupled to a pair of generally opposing jaws. Movement of the inner member relative to the outer body pivots the jaws open and closed. The jaws are configured for receiving and supporting a malleable surgical clip which is insertable within an internal retention groove within each jaw. Each groove has an undercut portion at its distal end which is configured to mate with an outwardly tapering portion at the distal end of each clip. When forced distally in the grooves, the distal ends of the clip are moved into the undercut portion and the clip retained within the groove. The clip may then be compressed and opened any number of times to facilitate its insertion, placement and removal.

    • An asymmetrical obturator adapted to be coupled with a trocar and to provide the trocar with characteristics for penetrating tissue forming a body wall. The obturator includes an elongate shaft which extends along a longitudinal axis between a proximal end and a distal end. The distal end of the shaft includes a first and a second longitudinal shaft portions, the first of which is distally configured with a sharp edge. The second shaft portion generally opposes the first shaft portion and is configured with a blunt distal tip. The first shaft portion and the second shaft portion are longitudinally slidable relative to each other between a cutting position and a safety position. In the cutting position, the second shaft portion is moved proximally relative to the first shaft portion to expose the sharp edge of the first shaft portion. In the safety position, the second shaft portion is moved distally relative to the first shaft portion to cover the sharp edge of the first shaft portion. A biasing element maintains the obturator in the safe position and an actuating mechanism allows the obturator to be temporarily moved into the cutting position.
