    • A laparoscopic insufflation device is provided in the configuration of a coil with a blunt tip. The device is capable of passing through the abdominal wall without cutting tissue, and exiting the abdominal wall substantially parallel to the inner surface. While rotation of the coiled device results in forward movement through the abdominal wall, a counter force can be applied to the device to create a safety space between the wall and the interior organs. With the blunt distal tip, parallel exit angle, and safety space, there is substantially no threat to the interior organs during placement of the device. Further space can be generated with the use of pressured gas to produce an abdominal cavity for the subsequent placement of trocars. By rotatably attaching the coiled insufflation device to a trocar, the advantage of a counter force can be used not only to establish the safety space but also to pull the trocar into the abdominal wall with a counterforce which resists tenting.

    • A tamponade trocar includes an elongate balloon having a closed distal end and is adapted to expand from a small diameter to a large diameter. A cannula is positioned at a proximal portion within the balloon lumen. A rigid stylet is removably positioned within the balloon and cannula lumens. A distal end of the stylet supported balloon is inserted into a perforation in a body wall and advanced into a body cavity while the balloon is unexpanded. The balloon is expanded and the cannula is advanced to a distal portion of the balloon lumen and across the body wall. At this stage, the perforation is dilated and in compressive tamponade. The proximal portion of the balloon may be removed and a seal housing may be coupled to the proximal end of the cannula. The distal end of the balloon may be punctured and opened, making the trocar ready for use.

    • A trocar adapted to extend across the body wall into a body cavity, forming a seal around an instrument inserted through the trocar, comprises a cannula having a first axis extending between proximal and distal ends. A valve housing attached to the proximal end of the cannula forms a working channel with the cannula and includes a rigid housing portion and a flexible housing portion. First and second septum valves are formed in an end wall of the flexible housing portion and form separate valve sets with associated zero closure valves disposed within the valve housing. The valve sets are sized and configured accommodating different ranges of instrument diameters. The first and second septum valves are offset from the axis of the cannula in normal positions but are movable to positions more aligned with the axis of the cannula, due to elastomeric characteristics of the flexible housing.

    • A surgical clip applier for use in applying ligation as well as fixation surgical clips or fasteners to a vessel or other tissue at a surgical site. The clip applier includes an elongated hollow outer body having a proximal end attached to an actuating mechanism and a distal end. A pair of generally opposing jaws are coupled to the distal end of the outer body and are configured with guide slots for receiving a surgical clip. Means are provided for advancing a clip from within the outer body into the guide slots. Movement of the actuating mechanism moves the jaws between an open position, a first closed position and a second and fully closed position. The jaws are placed over a section of tissue and closed to the first closed position such that the tissue is compressed. The clip site is tested for proper ligation, and if acceptable, a clip is advanced into the jaw members. A pair of shims are moved distally within the elongated outer body and inserted between the guide slots and the clip such that the clip is compressed over the tissue. The jaws are then moved to the second closed position to fully compress the clip over the tissue and the shims withdrawn. A V-shaped surgical clip of the present invention is provided in a semi-closed configuration to minimize the required overall outer diameter or width of the surgical clip applier.

    • A surgical device is disclosed for treating and removing obstructing material from within a vascular conduit or other body passageway. The device comprises a catheter device having an expandable and collapsible outer mesh sleeve disposed at a distal end. The outer mesh sleeve is configured such that when expanded in the obstructed conduit or passageway, a portion of the obstructing material is forced through a plurality of openings in its outer surface and thus, into the outer mesh sleeve. An inflatable and deflatable balloon may be coupled to the catheter and used to radially expand the outer mesh sleeve into the obstructing material. A treatment element disposed within the catheter device is moved against the inner surface of the outer mesh sleeve to treat and remove any obstructing material with which it contacts. The treatment element may be configured as a blade for physically removing the obstructing material, or as an electrode for electrosurgical treatment. After treatment or removal of the obstructing material, the outer mesh sleeve is collapsed and the catheter device removed from the body conduit.
