    • Apparatuses and methods for inserting elongated fixation elements into screw implants include one or components for visualizing the insertion process from a unique vantage point. One apparatus features a steering apparatus for adjusting the position of a receiver element on a screw implant, so that an elongated fixation element, such as a rod, can be inserted through a transverse passage in the receiver element. The steering apparatus may include an extension tube and a steering control member coupled to the extension tube. The extension tube may be configured for coupling to the receiver element, and the steering control member may be configured for coupling to the extension tube. The steering control member may include an opening for receiving an endoscope. The endoscope may be used to visually monitor the elongated fixation element as it is inserted into the transverse passage of the receiver element.

    • A surgical ratchet tool and system along with associated methods are disclosed. The surgical ratchet tool can include a relatively simple encompassing member that included a pawl extending into the opening for directly contacting a nut during surgery. The ratchet tool and system can be used for various types of surgery in which easy sterilization and a reduced profile for a ratchet tool is desired. For example, the ratchet tool can be used during surgery for adjustment of a spinal implant, where manipulation of a nut located on a downtube connected to a screw and/or spinal rod is desired. The encompassing member and pawl can be constructed/molded as a single piece unitary structure. A first handle and possibly a second handle can extend from an outer periphery of the encompassing member.

    • An articulating assembly includes a first elongated element for attachment to a first anatomical region, and a second elongated element for attachment to a second anatomical region. In at least one embodiment, a coupling connects the first and second elongated elements. The coupling includes a moveable joint configured to allow polyaxial movement of the first elongated element with respect to the second elongated element. The assembly further includes a locking mechanism. The locking mechanism is operable in an unlocked condition to permit polyaxial movement of the first elongated element with respect to the second elongated element, and a locked condition to immobilize the movable joint and fix the position of the first elongated element with respect to the second elongated element.
