To this end, the present invention comprises: a guide which is fixed to the outer circumferential surface of the femur and allows a center pin, a side pin, and a core decompressor of a drill handpiece to pass through to an expanded necrotic lesion; a guide body which attaches to and detaches from the guide, and in which a handle is provided on the outer surface of a block, a center hole is perforated from the front surface center of the block to the rear surface center, and side holes are perforated at a predetermined tilt angle with respect to the center hole, wherein the side holes are arranged radially on the front surface of the block and aligned with the center hole on the rear surface of the block; and a leading body which is installed in the guide and presses so as to move the center pin, the side pin, and the core decompressor of the drill handpiece from the procedure position of the femoral head toward the necrotic lesion via the guide body and the guide.
