Provided are an image processing device which can accurately determine ulcerative colitis by using a medical image, and an operation method for same. The image processing device (16) includes a processor, wherein the processor determines whether to perform image processing on the medical image on the basis of an image capturing condition of the medical image and/or an image analysis result obtained by analyzing the medical image. In addition, with respect to the medical image for which the image processing is determined to be performed, at least one among: calculating an index value pertaining to a stage of the ulcerative colitis on the basis of concentration of superficial blood vessels, intramucosal hemorrhage, and extramucosal hemorrhage obtained from the medical image; determining the stage of the ulcerative colitis; or determining remission or non-remission of the ulcerative colitis is performed.
