The invention discloses a measurement device and a right-angle coordinate system measurement method of a crack form of a concrete bridge. The device comprises a track and a trolley, wherein a first magnetic sensor is arranged at the top of the trolley, a front ball screw pair and a rear ball screw pair are arranged on the trolley, two racks and a second motor driving the racks are arranged on a sliding block of each ball screw pair, and a second magnetic sensor is arranged at the extension end of each rack; and the trolley is further provided with a longitudinal ranging device used for measuring longitudinal displacement of each sliding block and a transverse ranging device used for measuring transverse displacement of the extension end of each rack. The key to the method is to acquire theactual width of each measuring point and a width center line of a crack so as to obtain a contour line of the crack. Through the device and the method, the tendency and contour of the crack can be precisely represented.
